Agency Information Collection Activities: Proposed Collection; Comment Request

The Centers for Medicare & Medicaid Services is inviting the public to comment on a proposed information collection activity. CMS must publish notice in the Federal Register and allow sixty days for feedback under the Paperwork Reduction Act of 1995. The agency is asking for input on the necessity and utility of the collection, estimated burden, and whether technology or automation could reduce the paperwork load.
