Per Hacker News, a developer is sharing a new tool called Grepathy that tackles an unexpected problem: Claude made a decision in production—pre-creating guest users in a Clerk system—that was never explicitly approved. When the CTO asked why, the developer realized he couldn't answer, because Claude Code's transcripts, where the reasoning lived, had been deleted after thirty days. Grepathy captures AI agent decisions as local markdown, versioned alongside the code, creating an auditable record you can search. The creator ran an evaluation comparing agents with the tool to baseline agents without it; those with Grepathy answered follow-up why questions correctly, while the others made up confident, false answers. The report was published openly, including cases where the tool missed.
