What happens when your hospital is also your insurance company? We’re starting to see the effects

Today, nearly one-third of hospitals in America now own insurance plans—sometimes called "payviders." These organizations both decide when to provide treatment and how much to pay for it. The Conversation reports that hospital-owned Medicare Advantage plans now cover roughly one in six beneficiaries in that program. Research shows benefits: better coordination reduces red tape, and these plans show improved outcomes like fewer readmissions and complications. But concerns remain. Some hospitals may encourage doctors to record more diagnoses to trigger higher government payments, inflating taxpayer costs. And hospital-owned plans charge higher premiums than competitors. Researchers are still working to understand whether those premiums reflect genuine quality improvements, anti-competitive behavior, or a mix—and what that means for patients and public spending.
