Telstra blames missing software update and undocumented design change for massive outage

Per The Guardian, Australia's Telstra experienced a massive nationwide outage last week affecting mobile services, trains, and retailers. The company has now blamed the cause on a missing software update for a critical timekeeping system, along with an undocumented design change its maintenance teams didn't know about. Telstra stated it had redundancy in place — yet that redundancy failed to prevent the outage. Chief executive Vicki Brady is now facing a Senate inquiry over the incident.
